Job Summary
The Community Licensed Practical Nurse, in collaboration with the multidisciplinary team, performs assessments, plans, implements and provides personal and nursing care to clients.  Administers oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.  The scope or extent of the skills that the LPN performs varies according to the predictability of the outcome for a given client population.

Top 4 Key Responsibilities

  • Assesses and monitors client/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate, encourages client participation in activities of daily living.  Establishes and maintains a therapeutic relationship with the client and significant others through the use of interpersonal and interviewing techniques ensuring client choice and autonomy in decision-making and care planning.
  • Shares knowledge provides direction, assignment and supervision to unregulated care providers as appropriate.
  • Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences. Advocates for the interest of the client and significant others.  Collaborates with Community Care Clinician as required.
  • Administers oral, subcutaneous and intramuscular medications including PRN medications in accordance with organizational and pharmaceutical policies and procedures.
  • Provides maintenance of peripheral intravenous infusions by assessing the intracath insertion site, monitoring flow rate and discontinuing peripheral IV access when ordered by the physician.

 Job Qualifications

  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
  • One (1) year of recent, related experience; or an equivalent combination of training and experience.
  • Current valid BC Driver’s License.
