The RN - Team Lead will provide and facilitate safe, quality, client-centered health care. As a practicing nurse, the Team Lead will also provide additional supports, including but not limited to, coaching and mentoring of staff, education as needed and reviewing referrals.
Reviews requests for service, assessing for risk to client and service provider, and the appropriateness of care and assigns to appropriate care provider.
Liaises with Managers and Educators to optimize client and service provider experiences.
Acts as a resource to funders, the Client Service Associates and service providers to ensure safe and effective quality care.
Responsible to teach and delegate tasks and functions to different levels of workers and to family members and others in accordance with the standards and scope of practice as determined by VON and the College of Nurses of Ontario.
Comply with all standards, policies, procedures and legislative requirements.
This role does not replace the clinical expectations, resources and accountability held by the individual nursing members of the team, but facilitates the development and optimization of clinical expertise within the self-directed care team. This role compliments that of the Branch Clinical supervisor/resource.
Current registration as a Registered Nurse in the province of Ontario
Minimum 3 years experience in Community Nursing
Fundamentals of Hospice Care mandatory
Capce Certificate mandatory or to be obtained within 3 months
Demonstrated skill in observation, independent assessment, health teaching, communication and interpersonal relationship building
Solid knowledge and experience with Advanced Nurisng skills, best practice and advanced competencies including IV, CVAD, Wound care and palliative care..
Demonstrated coaching, mentoring and leadership skills
Solid oral and written communication skills and problem-solving abilities
Valid driver’s license, insurance and access to a reliable vehicle
Ability to speak French is an asset in French Designated areas
A current and original copy of a satisfactory Criminal Records Check is required upon the start of your employment
A Vulnerable Sector Search and/or Child Abuse Registry Check may be required
